Join the Oswego County Health Department (OCHD) and learn how to manage chronic disease this fall.

The Chronic Disease Self-Management Program begins on Aug. 27 and continues from 5 to 7:30 p.m. every Tuesday through Oct. 1 at the Nick Sterio Public Health Clinic, 70 Bunner St., Oswego. The free program is open to county residents.

Chronic diseases are persistent health conditions that require ongoing medical attention. They can limit daily activities, shorten lives and reduce quality of life.

This six-week program is crafted to help empower those living with a chronic health condition to better control their symptoms and enable them to live life more freely. The interactive, peer-led series welcomes those who live with chronic disease as well as those who care for someone living with chronic disease.

The program will help all participants learn skills to manage chronic health conditions such as:

  • arthritis
  • asthma
  • cancer
  • chronic obstructive pulmonary disease (COPD)
  • Crohn’s disease, irritable bowel syndrome (IBS) and other inflammatory diseases
  • cystic fibrosis
  • depression or post-traumatic stress disorder (PTSD)
  • diabetes
  • eating disorders
  • heart disease
  • high blood pressure
  • obesity
  • osteoporosis
