ARISE is pleased to announce a new Diabetes Self Management Program for people with disabilities who are on Medicaid. The six-week workshop is free and will take place once a week starting on Jan. 15.

Classes are designed to help participants learn the skills needed in the day-to-day management of diabetes, incorporate healthy lifestyle changes, create action plans, and teach individuals what questions to ask their health care providers.

The workshops will be offered at the ARISE Oswego office, 9 Fourth Ave., and will be led by certified peer-leaders, Shane Hoey ( ARISE Medicaid Service Coordinator) and Jim Karasek (ARISE Manager of Independent Living Services).

To sign up for the workshop or to learn more, contact Shane Hoey at 342-4088 x222 or Jim Karasek at 342-4088 x206 or register online at: https://www.ceacw.org/programs/dsmp and click on tab select a workshop.
